ANOTHER CHANCE HOME HELPERS is a subsidiary service of Another Chance Customized Services Inc. Home Helpers is a home assistance services that is customized to meet all your needs. Our services are available to you both long and short term assistance, regardless of your age, disAbilities or illness.

ANOTHER CHANCE HOME HELPERS is licensed, bonded and insured, and all of our in-home care providers are carefully screened with extensive background checks, including motor vehicle record history, social security validation, and diligent reference checks.

Asking for help can be difficult, but with the assistance of ANOTHER CHANCE HOME HELPERS you are on the right path to comfort. Our dedicated staff members work one-on-one with children, adults and seniors to restore independence and the ability to complete and improve day-to-day activities.

CLS

Community Living Support provides companionship, errands & transportation, status reporting to family, friends visits, light housekeeping, salon & barber visits and more! This is an offsite service where your loved one is visited or transported in order to integrate with the community. Daily or weekly activity is made accessible to supported persons by experienced, professional staff members that can provide a safe and rewarding day for them.

Our staff consists of Certified Nurse Assistants, Personal Care Assistants, Companion Sitters and Nurses. They are all BLS CPR and Crisis Prevention Intervention certified. State required training is given regularly to update and educate the support team to better serve the community,

GAPP

GAPP is a Medicaid program for medically-qualified children under the age of 21. Children of families eligible for Medicaid can apply for GAPP at any time and must meet the same level of care necessary for admission to a hospital or skilled nursing facility. GAPP is managed by the Georgia Department of Community Health (DCH).

CCSP

Georgia Medicaid's Community Care Services Program (CCSP) operates under the Elderly and Disabled Waiver. CCSP is a program designed to help frail older adults remain living in their own homes or communities and to receive services in those locations as an alternative to nursing homes.

SEI (SUPPORTED EMPLOYMENT/JOB COACHING)

Supported employment refers to service provisions wherein people with disabilities, including intellectual disabilities, mental health, and traumatic brain injury, among others, are assisted with obtaining and maintaining employment.

MEDICAL SUPPLY

We provide SME equipment for those applicable.

Nursing Services

Skilled nursing care refers to a patient's need for care or treatment that can only be performed by licensed nurses. This type of care is usually offered in hospitals, assisted living communities, Life Plan Communities, nursing homes and other certified locations

S.O.U.R.C.E

Service Options Using Resources in a Community Environment (SOURCE) links primary medical care and case management with approved long-term health services in a person’s home or community. SOURCE operates under authority of the Georgia Elderly and Disabled 1915c Medicaid Waiver and provides Home-and-Community-Based Services to frail elderly and physically disabled people who meet the Intermediate Nursing Home Level of Care
